Abstract Many patients with anti-Yt a receive multiple transfusions of Yt(a+) red blood cells (RBCs) no ill effects. However, has been implicated in hemolytic transfusion reactions. Antibody identification typically determines specificity antibodies and their clinical significance to justify requirements for antigen-negative when clinically significant are involved. Occasionally, antibody is variable significance. Variability characteristic that may affect the management such patients. This case reports outcome an incompatible 83-year-old female patient , -D, -C, -Le ab -HI who was admitted hospital severe urinary tract hemorrhage fever. The transfused 1 crossmatch-incompatible group A, Yt(a+), D–, C–, E–, S– RBC unit emergency medical event. During time, exhibited chills, shivering, tachycardia. Decreases hemoglobin hematocrit were noted. Laboratory parameters hemolysis, as total bilirubin, direct lactate dehydrogenase, increased. Based on laboratory evaluation, it concluded had acute reaction caused by . successfully treated antipyretics, antihistamines, corticosteroids. Urinary hemorrhaging stopped. Anemia additionally improved parenteral iron supplementation, further not required. Immunohematology 2021;37:13–17
